A warrant for the arrest of a Eureka Springs man accused of negligent homicide and driving while intoxicated has been issued, according to court documents.The incident took place on Saturday, Sept. 27, 2025, on State Highway 23 in Madison County.According to a court document, Jason Koeffler, who was driving a 2003 Harley-Davidson motorcycle, attempted to pass on the left lane in an attempt to avoid colliding with a slow-moving vehicle.WCSO responded to a fatal UTV accidentThe document said, Koeffler lost control of the motorcycle. Koeffler and a passenger on the motorcycle were "separated from the motorcycle with the passenger sustaining fatal injuries."A preliminary report from the Arkansas State Police says the motorcycle went into a ditch, then got back on the road, where it landed on its side.The preliminary report identified the passenger as 35-year-old Amanda Stonebraker from Goff, Kansas. Stonebraker was pronounced deceased at the scene of the incident by the Madison County Coroner.Prairie Grove police is investigating a fatal accident involving 19-year-old driverNeither Koeffler nor Stonebraker was wearing a helmet at the time of the accident, according to the court document. At the time of the incident, no statements were taken from Koeffler due to his sustaining injuries.A blood sample was taken from Koeffler at the time. The results showed a blood alcohol concentration of .082, and he had THC present in his system, according to the court record.
