The incident involved Randall Hoffman and Amanda Lindzy, both residents of Churubusco, colliding at the intersection of Main and Liberty Streets. Hoffman was driving north on Main Street when his vehicle crossed the center line for unknown reasons, leading to the head-on collision. Emergency responders arrived at the scene promptly, with Hoffman tragically pronounced dead and Lindzy transported to a nearby hospital.
