Coroner identifies driver killed in Thursday crash on West Jefferson Boulevard

Video shows initial coverage of the fatal crash FORT WAYNE, Ind. (WANE) - The Allen County Coroner has identified the driver who died Thursday after a collision near Swinney Park as a Kendallville woman. The coroner identified her as Mikaela Tish Forker, 27. The crash happened Thursday morning in the 2300 block of West Jefferson Boulevard, causing a stretch of the heavily-traveled road west of downtown to be temporarily shut down. Fort Wayne police determined Forker's car veered onto the opposite side of the road for some reason, hitting another car head-on. The other driver was initially trapped, but was rescued with only minor injuries, the FWPD said. Forker, however, had life-threatening injuries and was taken to a hospital where she later died. The coroner determined her death to be an accident, which is the standard ruling for crashes.
