Fatal Crash Interstate 15 Freeway, Jurupa Avenue in Ontario

ONTARIO, Calif., (April 28, 2026) — A fatal two-vehicle collision happened early Tuesday on the Interstate 15 Freeway south, near Jurupa Avenue in Ontario. The tragic incident unfolded at approximately 4:47 a.m. on April 28, 2026, according to the California Highway Patrol. The crash involved a white sedan and a work truck, blocking all lanes except the far right lane. Emergency responders, including firefighter/paramedic D. Paullins, arrived on the scene where one individual was partially ejected from their vehicle and sadly was pronounced deceased at 5:06 a.m. Next, the San Bernardino Sheriff’s Office was notified to start coroner procedures.
