Second man accused in deadly West Valley collision

A second man, Regino Mateo Mendoza from Magna, has been charged in connection with a fatal three-car crash in West Valley City that resulted in the death of a 25-year-old woman from Tooele. Mendoza faces charges including automobile homicide, negligently operating a vehicle causing serious injury, obstruction of justice, driving without insurance, and crossing the median. The incident took place on the Bacchus Highway, where a Toyota Corolla driven by Madison Wagner was struck by Mendoza's Mazda, leading to a chain reaction crash involving another vehicle. Mendoza initially tried to flee the scene but collapsed soon after. He was found to have a blood-alcohol content level of 0.104%. The driver of the third vehicle involved, Oswaldo Chavez Segovia, also attempted to leave but was located by police with K-9 units. Chavez had previously been charged with offenses including leaving the scene of an accident involving death, failure to stop at a serious injury accident, obstruction of justice, making a false statement, and driving without a license.
