NTSB's report on Francis Scott Key Bridge incident
A small label placed incorrectly on a wire during the construction of a massive cargo ship may have led to a series of events culminating in the ship colliding with a bridge and collapsing into the water nearly a decade later. The National Transportation Safety Board is conducting a public meeting to determine the root cause of the incident involving the container ship Dali hitting the Francis Scott Key Bridge, resulting in the deaths of six individuals. The NTSB revealed that an incorrectly positioned label on a signal wire during the ship's construction prevented a proper connection, triggering a blackout and subsequent operational failures onboard the vessel. This led to the vessel losing steering control, power, and essential equipment, ultimately resulting in the tragic collision with the Key Bridge. The NTSB is working to ascertain the primary cause of the crash and will vote on a final report following the meeting. The incident has highlighted the need for bridge vulnerability assessments, as similar risks may exist for other critical bridges across the United States.
