Driver charged with car manslaughter after fatal Kyhv Peak crash, left injured person at scene

A driver from Orem, Zachary Ingalls, has been taken into custody for the tragic death of Kortnie Benson Barajas following a rollover incident in Provo. Ingalls, 31, is currently facing charges of automobile homicide after the incident that took place on Kyhv Peak Road on Aug. 1, 2025. When Ingalls sought medical treatment for injuries from the crash, he allegedly left behind Barajas, who was found deceased in the crashed vehicle. The recent arrest of Ingalls comes after it was found that tests on him following the crash showed the presence of THC, opiates, and alcohol in his system. Ingalls had a prior arrest and conviction related to a car accident in 2022, where THC was also detected in his blood. Following his arrest on Thursday, Sept. 11, Ingalls is being held in Utah County Jail. Barajas' family expressed deep sorrow over the loss of their loved one, revealing she was likely returning from a camping trip when the crash occurred. Barajas is survived by her husband and children, leaving a lasting impact on those who knew her.
